You have the right to see the information held about your care. Because a record is highly sensitive, every request goes through identity checks, never through a general web form or email.

How a records request usually works

  1. Say what you need

    A whole record is rarely needed. Naming the period, the department or the specific letters or results usually makes a request much faster to fulfil.

  2. Prove who you are

    Identity is checked before anything is released. This step protects you: it is the reason a record cannot be requested by email alone.

  3. Requesting for someone else

    You will need the person's written authority, or legal authority to act for them. Requests about someone who has died follow separate rules.

  4. Receiving the record

    Records are released through a channel confirmed with you, not by ordinary email. Ask what format is available if you need large print, an accessible file or a translation.

  5. If something looks wrong

    You can ask for a correction to be considered and recorded. Say clearly which entry you mean and why you believe it is inaccurate.
